Please complete the following steps for your discussion post and response.

For a program to be successful in the long-term, outcomes or results must be replicated with consistency. Consider the following details as the basis of this discussion:

A chronic disease care management program is proposed for implementation in your hospital.

You will be in charge of collecting the program’s data and analyzing efficacy.

You will also be responsible for recommending the program’s continuation or discontinuation based on it’s success or failure to improve patient outcomes and control costs of care.

Questions:

What data points will you track, measure, and evaluate?

Discuss the concept of replicating results for this program. What benefits and risks are associated with continuing the program while replication is attempted?

Discuss how you will quantify the program’s results for an official recommendation to hospital leadership. What determines success, failure, etc?
