Dear Dr. Stewart,

Mr. Richards presented to our office on December 16, 2012, with complaints of nocturia, hesitancy, and a weak urinary stream. He has a history of right-sided orchiopexy for cryptorchidism (which was performed in 1965), a vasectomy performed in 2001, and a TURP for BPH performed in 2006. His PSA was 4.4. DRE revealed a mildly enlarged prostate gland with a 1.5 cm nodule of the right lobe. TRUS and biopsy was performed. The patient was diagnosed with adenocarcinoma of the prostate. The patient elected to undergo robot-assisted radical prostatectomy, which was performed on January 25, 2013. The patient has had an unremarkable post-operative course. No urinary incontinence or ED was reported. His weight is stable at 209 pounds and blood pressure is 122/82. DRE is normal. His PSA was 0.3 as of February 27, 2013. I would like to continue seeing him on a yearly basis for PSA and examination. Thank you for your kind referral.
