Sample Notes Critique

Please see attachment and review sample SOAP note documenting a therapy session with a client named Alex. Carefully evaluate each section of the note—Subjective, Objective, Assessment, and Plan—using the SOAP model as your guide. Consider whether the information is complete, clear, and appropriately detailed. Identify what is done well and areas that need improvement. Write a critique that is 1-2 pgs long, providing specific, constructive feedback on how the note could be enhanced. Offer examples and actionable suggestions to improve the completeness and clarity of each section. Conclude with a brief reflection on the importance of proper documentation and what you learned from this exercise. Your critique should be professional, supportive, and focused on helping improve the quality of documentation. 

Revised SOAP Note with Intentional Errors:

Alex seemed happy today, although he was very energetic. His mother mentioned that he didn’t sleep much last night. During the session, Alex was present for 30 minutes. He mostly played with his toy car and looked briefly at a book. At one point, he hit when asked to transition to another activity.

Overall, Alex seemed to enjoy himself, but he was a bit fidgety. His aggressive behavior is likely due to being tired. The current plan is to stick with the existing behavior intervention plan (BIP), and no changes are needed at this time.
