Mod 4 case strategic

You identified one health program that is being offered by an organization in your community in the Module 2 Case Assignment. Use the same health program to:

1. Use one of the sustainability assessment tool from the required reading to assess the health program sustainability you identified Module 2 Case. Summarize the results of your assessment.

2. Identify which domains need improvement and describe ways to address those gaps.

3. Describe the benefits to assessing a sustainability plan.

Length: 2–3 pages double-spaced, excluding the cover page and the reference list.

Introduction

In my previous assignment in the Module 1 SLP, I selected obesity as one of the most chronic illnesses affecting most people in the United States. Obesity is a problem that is affecting the health of Americans. Obesity has become a significant issue in the United States, an epidemic that needs to be addressed. Obesity is excess body fat that can cause heart disease, stroke, diabetes, hypertension, and certain types of cancer (Kaplan et al., 2019). There are many causes of obesity, and one of the leading causes is lack of exercise. People who do not exercise tend to eat more food and gain weight. Another reason for obesity is that people overeat food because they do not know how much they should eat each day or what foods are good for them.

Q1. Identification of the organization

The best organization to fight obesity in my area is the American Heart Association. The organization fights obesity holistically by promoting heart health. The organization has vast experience in managing, preventing, and eradicating obesity since it was formed in 1924 (Dunlay et al., 2019). Six cardiologists initially formed the organization to eradicate obesity by managing various heart diseases.

Currently, the organization is considered an international organization that operates locally in the United States and internationally in other countries in Europe and other regions. American Heart Association provides programs focusing on healthy living through education, research, and advocacy efforts to prevent obesity and cardiovascular disease (CVD). The AHA has developed guidelines for weight management that include; increasing access to health care services, including counseling about weight management options and screening for obesity. It also promotes healthy eating by offering food packages with greater variety, lower sodium levels, and lower fat content. The association’s
mission is to control the risk factors for obesity, cardiovascular disease, and stroke, including high blood pressure, high cholesterol, and smoking. The American Heart Association’s
vision is to make cardiovascular disease prevention the nation’s number one priority so that everyone starts living longer and healthier lives.

Q2. Health Program

The organization’s program is education, research, and advocacy to prevent obesity. The program’s goals are to increase awareness and knowledge about the relationship between heart disease and obesity; to provide programs and tools that can be used by schools, community organizations, faith-based groups, businesses, and families. The organization also increases access to healthy food choices; assists those who are obese or at risk of becoming obese through screening, counseling, and referrals. It has also developed effective strategies for promoting physical activity among youth and encourages community leaders at all levels of government to adopt policies that promote physical activity in their communities.

The goals and objectives of the program are to educate the public about heart disease and stroke, provide resources for patients, caregivers, and families affected by heart disease, improve risk factor management programs, and support community-based efforts to combat obesity through education, research, and advocacy. The AHA’s program aligns with its mission and vision since it is designed to educate people about heart health associated with other chronic illnesses such as obesity, diabetes, and cancer. The organization also works with communities to provide resources for families who want better care of themselves.

Q3. Developing my own program goals and objectives

Obesity in the United States affects people of all ages. I would implement a program promoting body exercise among children and adults. The program will be
an Adult fitness and
young boys and girls exercise program.
Adult fitness will conduct as a series of classes, usually three times a week. Each class consists of approximately 45 minutes of aerobic exercise and strength training. The instructors will teach proper form and technique to help participants maintain their health and lose weight. This program aims to help adults maintain a healthy lifestyle by increasing their physical activity levels and improving their health.

The young boys and girls exercise program will promote a healthy lifestyle among children ages 8-12 by teaching them how to be physically active. The program’s goals and objectives are to help young boys and girls develop healthy habits and improve their physical fitness levels to become more active and fit in adulthood. Participants must participate in daily physical activity such as walking, biking, playing sports, or dancing for at least 20 minutes each day. The program aligns with the organization’s goals and objectives since exercise promotes weight loss and reduces cholesterols and activeness, preventing obesity and other heart diseases.

Q4. Competencies

The first
core competency is to perform effectively in an interprofessional team. Interdisciplinary teams are professionals from various disciplines who work together to solve problems (Flaherty & Bartels, 2019). As a member of an interprofessional team, a public health professional must have the ability to recognize when he or she may need support from another professional or take charge of the situation. A public health professional must be able to communicate effectively with other professionals, including but not limited to physicians and nurses, social workers, dietitians, and physical therapists.

The second core competency is promoting, developing, and using an integrated clinical-community care plan. The competency includes all levels of care for individuals with obesity and their families. It includes multiple stakeholders, such as community leaders and policymakers, who plan programs to address obesity at multiple levels, including individual behavior change, family education, and community engagement strategies. A public health professional must be able to identify opportunities at each level within his or her community that will help reduce obesity among those who may be affected by it.
