Write a script for me to record with someone for the zoom portion

Using Zoom, record a role play of a portion of your service planning meeting. You must work with another individual to complete an 8-15-minute Zoom recording of the introduction process and completion of the first service goal with your “client.” You will assume the case worker role and your partner will be the client. Use the attached service plan template to record the information you cover in the role play.

Have the “client” complete the attached volunteer informed consent form prior to recording your service planning meeting. Submit this form to your instructor.

Once completed, review the video, using self-reflection, consider your work in the role and answer the following questions:

SOC-445: Ruth Case Study For Assignments
Persis was Ruth’s case manager at a substance abuse center. Ruth reports that while in her second
year of college she began using alcohol as a coping mechanism to deal with symptoms of
depression, a lack of motivation. Ruth came to drug treatment when she started to see her life
spiral out of control. Ruth admitted to drinking 4-5 glasses of wine daily, for 2-3 days per week.
Ruth stated that “waking up for work is an arduous task, missing 1-3 days weekly. During the
initial interview Persis clarified, using reflective listening skills, that Ruth drank at least 4-5 days
weekly.
Ruth was raped by her father when she was 22 and had moved away from the family home to get
away from her father. After the rape he threatened to beat her if she told anyone about the rape.
He had been sexually molesting Ruth prior to that for seven years, beginning when Ruth was 15.
Ruth became pregnant and was stalked by her father, forcing her to move several times. She
went to an obstetrician early in the pregnancy. Feeling that there were problems with the fetus,
based on testing, the doctor ordered an ultrasound and learned the fetus appeared to be forming
without essential vital organs. Ruth was in anguish over this and over the continued harassment
and threats from her father. Many, many tests were done to be sure before the decision was
made.
Ruth reported to Persis that she had an abortion 6 years ago. For Ruth this was a difficult
personal decision she made with her physician. Ruth was reluctant to share this information with
Persis, feeling she did not know Persis well enough. For Ruth the circumstances surrounding the
abortion are embarrassing. Ruth mentioned “the abortion” in passing while describing another
aspect of her life. Persis immediately wanted to know more. Ruth gave her some of the details
but was uncomfortable. Persis pressed for more, leading Ruth to feel that Persis would not help
her if she did not tell Persis everything.
Persis then lectured Ruth about abortion as “the worst sin against God”. She assured Ruth that if
she had had faith the abortion would not have been necessary. She went on to think up as many
different ways Ruth could have gotten away from her father before it reached this point, none of
which were based on the real facts of Ruth’s situation at the time, which Persis did not know.
Persis wanted to bring in Ruth’s father so that “the two of you can resolve your differences.”
Ruth said she did not want to deal with her father under any circumstances and she added that
she felt there were more than “differences’ between them.
During the initial interview Persis had Ruth sign a release of information form. The form did not
say what it was for, who was to receive it, nor did it have an expiration date. Persis said the form
would be used in case the agency needed “other records”. However, after Ruth left the agency,
Persis sent for Ruth’s medical records concerning her abortion. Persis was convinced that these
records would show the abortion had not been necessary and she planned to use the records to
confront Ruth when Ruth returned.
